Mount Gretna was founded in 1782 and the original town was named "Gretna Green". Mount Gretna was chartered as a town on October 6, 1795. The earliest settlers were Quakers who had come from Bucks County, Pennsylvania. The town was originally located on the eastern bank of the Potomac River, opposite what is now Georgetown, but relocated to its current location in 1868 when the railroad was completed.

The economy of Mount Gretna is mainly based on tourism and the health sciences. The city is home to the Potomac Heritage Museum which is a Smithsonian Affiliate. The museum has a collection of Americana, including artifacts from the Colt revolver and the first Ford car. The city is also home to the Walter Reed Army Institute of Research, which is the largest biomedical research facility in the world.
