Killingworth is a city located in Middlesex County, Massachusetts, in the United States. The population was 23,397 at the 2010 census.

According to the United States Census Bureau, the city has a total area of 4.5 square miles (11.5 km2), of which 4.5 square miles (11.5 km2) is land and 0.12% is water.

Killingworth is situated in the northeastern portion of Middlesex County at the eastern edge of the Boston metropolitan area. The city is bordered to the east by Dunstable and to the west by Stoneham. The city also has a small border with the town of Reading to the north.

The city is bisected by the Mystic River which forms its north-south boundary. The river separates the city into its central and eastern sections. The central section of Killingworth features a mix of residential, commercial and industrial areas. The eastern section of the city is largely composed of natural areas including sections of the Mystic National Forest and Little Pequot Pond.
